How to add a document request to a customisable form

Learn how to add required, optional, or conditional document requests to a customisable form.

You can add document requests directly within a customisable form. A document request can be required, optional, or conditional β€” appearing only when a client gives a specific answer to a previous question. Any files the client uploads are combined into the custom form PDF in their record.


Before you start

  • Administrator permissions are required.

  • To add a conditional document request, you need a question with multiple answer options (such as a Radios or Checkboxes question) to act as the trigger.


How to add a required document request

A required document request appears to all clients completing the form and must be completed before they can submit. Use this when a document is always needed, regardless of the client's answers.

  1. In the form builder, click Add item.

  2. Select Document request as the item type.

  3. Enter a message for the client (for example: "Please upload a copy of your passport or driver's licence").

  4. Optionally, add help text to give further guidance.

  5. Ensure the Required checkbox is ticked.

  6. Click Save.

  7. Click Publish to make the update live.


How to add an optional document request

An optional document request appears to all clients completing the form, but they are not required to upload anything. Use this when supporting evidence is helpful but not mandatory β€” for example, when a client may not have a document immediately to hand.

  1. In the form builder, click Add item.

  2. Select Document request as the item type.

  3. Enter a message for the client (for example: "Please upload any evidence of your source of wealth, such as a payslip or bank statement").

  4. Optionally, add help text to give further guidance.

  5. Untick Required to make the upload optional.

  6. Click Save.

  7. Click Publish to make the update live.


How to add a conditional document request

A conditional document request only appears to the client if they give a specific answer to a previous question. For example, only asking for a marriage certificate if the client confirms they have changed their name.

  1. In the form builder, hover over the question that will trigger the document request.

  2. Select Add conditional item from the options that appear.

  3. Select Document request as the item type.

  4. Enter a message for the client.

  5. Optionally, add help text to give further guidance.

  6. Set the condition: choose which answer triggers the document request to appear.

  7. Click Save.

  8. Click Publish to make the update live.

Conditional document requests are highlighted in yellow in the form builder so you can identify them at a glance.


What happens next

When a client completes the form, required document requests must be uploaded before submission. Optional document requests can be skipped. Conditional document requests only appear if the client's answer matches the condition you set. Any documents uploaded are added to the end of the custom form PDF in the client's record.


Important information

  • The Required checkbox is ticked by default when adding a document request. Untick it to make the upload optional.

  • Conditional document requests are highlighted in yellow in the form builder.

  • The triggering question for a conditional document request must have defined answer options, such as a Radios or Checkboxes question.

  • Uploaded documents appear at the end of the custom form PDF in the client's record.
